Substance Abuse vs. Substance Dependence That May Lead to Drug Rehab

The United Nations estimates that up to 50 million people worldwide may use cocaine, heroin, or other synthetic drugs on a regular basis. But how many of those people simply “abuse” these drugs, and how many are truly “dependent” on them? Is there any way to tell the difference between substance abuse and substance dependence? The two are distinct conditions and recognizing which of the two a person has can make all the difference in their rehabilitation.

Substance “abuse” is a broadly-defined condition whose parameters differ depending on who creates the definition. The phrase evolved from the term “drug abuse,” which many medical diagnostic tools, such as the World Health Organization’s International Statistical Classification of Diseases and Related Health Problems (ICD), no longer recognize. The Diagnostic and Statistical Manual of Mental Disorders (DSM), which catalogues all of the mental disorders recognized by the American Psychiatric Association, defines “substance abuse” as repeated drug or alcohol usage which, while not heavy enough to cause significant withdrawal symptoms, still causes problems in the user’s life.

Substance “dependence,” on the other hand, is much more clearly defined. The DSM defines substance abuse as a pattern of behavior wherein “an individual persists in use of alcohol or other drugs despite problems related to use of the substance [...] Compulsive and repetitive use may result in tolerance to the effect of the drug and withdrawal symptoms when use is reduced or stopped.” Substance dependence is thus seen as a more serious or more advanced form of drug usage, wherein the user takes the drug more regularly and feels withdrawal effects more acutely and may require drug rehab.

The difference between these two terms may appear trivial, but is of major concern to governments and treatment programs. Labeling a person’s drug usage as one or the other may offend him or her or make them less likely to comply with treatment in the future.
